Please bring the appropriate completed form(s) when you visit the Clerk's Office, or come prepared to complete the form(s), on your own, that are required for a passport book or card, depending on your needs. The standard passport application form DS-11 and all other necessary forms are available at the Clerk’s Office or can be downloaded from the Department of State.
